Refer and Earn

Step 1
Tell your family & friends about Hygeia HMO plans. Hygeia HMO plans cover individuals, families, senior citizens, businesses, and much more. Visit www.hygeiahmo.com to learn more about our plans.
When they are ready to buy the plan, they can make payments online. You (the referrer) can also fill this form with the contact details of the party being referred.

For Web payments, Ensure that the referee enters your Hygeia HMO Enrollee ID in the Referral ID field on the online form. (This will identify you as the referral of the sale and facilitate prompt rewards)

Click on “pay annually” to pay via the web. Once payment is confirmed by Hygeia HMO, the referrer will be contacted by a Hygeia HMO representative.

Refund: Global Healthbridge will refund any return based on request within 30days of purchase, less an administrative fee of 3%. Please contact info@globalhealthb.com.
Card: Member will be provided with a Membership Card. This card and other forms of identification should be carried by the Member ID to provide proof of the right to Eligible Services under the Membership Agreement. The discounts on the card may not be used in conjunction with any other discount plan or Plan.

Agreement: This agreement covers the terms and conditions of your participation (member) in the Global HealthBridge Discount Program when accessing healthcare providers in the United States. Your enrollment in the discount program, subject to payment of the specified premium, begins on the effective date and ends on the termination date shown on your Member Confirmation Letter or Member ID card.
